Free Consultations for Caregivers of People with Alzheimer's and other Dementias

Families and friends whose lives have been changed by Alzheimer’s or other forms of dementia face unique challenges. Caregivers play a significant role from the moment of diagnosis, and often do not receive the vital support they need to best care for their loved one. Through the New York State Department of Health Alzheimer’s Disease Caregiver Support Initiative, WJCS' Pathways to Care program provides professional assistance to individuals and families caring for a loved one with Alzheimer’s or other forms of dementia.

WJCS compassionate and highly trained staff offers:
  • Care consultation and clinical assessment
  • Individualized care planning 
  • Resource referral
  • Individual and group counseling 
  • Discussion/completion of advanced directives 
  • Referral to home care and respite services
