Healthy People, Healthy Carolinas
The Duke Endowment has released a Request for Applications for Healthy People, Healthy Carolinas (HPHC), a program to improve community health outcomes in North Carolina and South Carolina. This new opportunity is designed to support new or existing community coalitions in their efforts to improve population health through:
- Applying the Collective Impact framework.
- Enhancing community capacity to implement evidence-based interventions addressing physical activity/nutrition and chronic disease; and
- Monitoring performance metrics to spur improvement.
The overall aim of the initiative is to stimulate improvement in population health. This initiative is intended to drive improvement in outcomes and be transformative to community coalitions by enhancing the ability to build partnerships and applying new approaches to optimize community health.
Applications for HPHC in SC are due to The Duke Endowment by 5:00 PM on January 16, 2017, and the Request for Applications is here. Questions about the program should be directed to Lindsey Perret at 803.744.3507 or firstname.lastname@example.org.
If you are interested in applying for HPHC, you are required to have attended one of two scheduled webinars on November 22. If you did not attend one of the webinars, please reach out to Lindsey Perret for the webinar recording.
