Worldwide, integrated community care is seen as key to effective health and social services. The Transnational Forum on Integrated and Community Care (TransForm) is bringing policymakers, providers, and other partners together for four conferences exploring design and implementation of integrated community care. Transform is a partnership between GBF and several large European foundations including Compagnia di San Paolo (IT), La Caixa Foundation (ES), Fondation de France (FR), the King Baudouin Foundation (BE), Robert Bosch Stiftung (DE), the International Foundation for Integrated Care, and the Network of European Foundations.
"Building Stronger Communities through Integrated Community Care", TransForm’s third conference, took place in Vancouver in October, 2019. The Graham Boeckh Foundation, the Conconi Family Foundation and the St Paul’s Foundation co-hosted the event. Participants went on site visits to two Foundry locations as well as organizations serving Indigenous and immigrant communities. The conference had a particular focus on Indigenous communities and youth. Case studies, a synthesis report and video are available here
The final TransForm conference will be in held Brussels in March, 2020.
